Regarding Melian choosing her form for Thingol: The Maiar could assume various physical appearances, so perhaps Tolkien meant that Melian decided to stay with the physical manifestation in which she appeared to Elwe, as opposed to being free to change from one form to another, or to her formless fea.
I prefer to think that Melian assumed an Elvish-compatible form as she fell in love with Thingol.
So she 'chose' to be bound to that form, but had appeared in that form prior to Thingol coming upon her.
