Romance disappeared as a force in literature in the 17th century with the rise of empirical thought, rationalism, a theology based on analogy to the natural world and the advent of the bourgeois mode of realism, although it retained a slim foothold through pastoral.
Today we have still both gothic and historical romance, and romance is generally associated with the strange and mysterious, the adventurous, with the lure of foreign lands, with something slightly magical, with a story which refuses to be tied to the realist tradition and explores phenomena which are unusual, allegorical, symbolic.
Of course, we have True Romance, and the localization of the long tradition of courtship stories in our culture in romance settings, whether it be haunted homes, the wild west, or bleak, wind-swept shores.

Keep in mind that romances are about the girl getting her man in the end.
The characters can be traditional man/woman, or man/man, woman/woman, woman/ogre, etc, but the story must be about their romance.

 Differnt Genres and Approaches
As for how people in romance and science fiction approach their markets, the answer is that there is no 'one answer fits all.' We are not writing for the same audience, and what may look like a perfectly reasonable response for a romance writer is often going to appear unnecessary to someone who writes sf.
And even those cases of cross-genre stories -- like a time travel romance -- the way in which the book is written is going to depend on which aspect of the work is stressed, and which audience is the primary target.
As in all genres, there will always be writers who hold the middle line and write material that fits the medium market rather than striking out in new directions.

The book can even be about the romance and still not be a romance novel.

This is why, I believe, Diana Gabaldon started out shelved in romance, because her publishers thought (rightly, I think) that genre romance readers would be more tolerant of time travel and ghosts than sf readers would be of the romance aspects (even though they weren't genre romance aspects).
After all, mystery and romance are genres, on the one hand, and plots, on the other.
In the case of _S&C_, I think mystery is a plot, but Regency *is* a genre instead of a setting (though I'm not sure why _A College of Magics_ isn't fantasy set in Edwardian times, but then I don't know Edwardian from anything).

I have read a good number of romances, and I can honestly say that most are no more poorly written than the average thriller, mystery, or science fiction novel.
The romance divisions of many publishers help to subsidize the publication of "serious" literature that will only sell a few thousand copies.
